Summary

Clinical findings, diagnosis and prognosis are described in a 19 years-old boy with Sturge Weber Syndrome (SWS) associated with a Klippel-Trenaunay syndrome (KTS).

The SWS presented at birth with right facial angioma reaching the first and second branchs of trigeminal nerve (V1, V2), ophthalmologic abnormalities (buphtalmos, congenital glaucoma) and neurologic signs (seizure, hemiparesis, mental retardation).

The cerebral CT-Scan showed ipsilateral lepto meningeal angioma, calcification and cortical atrophy. Some degree of hypertrophy of both right superior arm and inferior leg suggestive of Klippel-Trenaunay syndrome was noted. Trabeculectomy and phaco-exeresis were perfomed in the right eye with additional beta-blocker drops to manage residual hypertony.

In despite of the early diagnosis and surgical treatment of glaucoma, the boy experienced a poor functional and neurologic prognosis leading to right eye blindness, recurrent seizure, hemiparesis and mental retardation.

Les manifestations cliniques, le diagnostic et le pronostic sont décrits chez un garçon de 19 ans avec le Syndrome de Sturge-Weber (SSW) associé au syndrome de Klippel-Trenaunay (KTS).

A la naissance, cet enfant a présenté à l’hémiface droite un angiome facial intéressant les deux branches du trijumeau (V1 et V2). Il a présenté également un glaucome congénital (buphtalmie) et des signes neurologiques (crises convulsives répétitives, hémiparésies répétitives et un retard mental). La tomodensitométrie cérébrale a montré un angiome leptoméningé ipsilatéral avec des calcifications intracérébrales. On a noté également une hypertrophie de deux membres supérieur et inférieur droits ainsi que du pied droit caractéristique du syndrome de Klippel-Trenaunay.

Une trabeculectomie et une phaco-exérèse ont été pratiquées à l’œil droit et complétées par un traitement hypotenseur oculaire (Bétabloquant). Malgré un diagnostic et un traitement chirurgical précoces du glaucome, l’évolution du cas s’est soldée par une cécité totale de l’œil droit, une hypertonie persistante, des convulsions répétitives mal contrôlées par les barbituriques, hémiparésies répétitives et une atrophie corticale cérébrale.
