RésuméSummary

Objectif et méthodes. Analyse de 17 dossiers de patients admis pour inconfort sacroiliaque durant une période de 7 ans, au service de Rhumatologie du CHU de Cocody. Les plaintes (lombo-fessalgies, sciatalgie ou cruralgie), les données de l’examen physique, celles de la radiographie standard, ainsi que le diagnostic présumé, ont été enregistrés.
Résultats. La fréquence de la pathologie sacroiliaque était de 0,55% (17/3067 admissions), avec prédominance du sexe féminin (14 femmes ; 82,35%) ; l’âge moyen étant de 25,58 ans. Les évènements gynéco-obstétricaux ont constitué les principaux facteurs favorisants (47,05%). La douleur de type inflammatoire (64,7%), siégeant à la fesse ou au rachis lombaire, irradiant à la cuisse (52,9%) et s’accompagnant d’une impotence fonctionnelle (82,2%) et inconstamment d’une fièvre (64,7%) a été la principale description. La triade physique a consisté en la manoeuvre du trépied positive (58,8%), l’appui monopodal positif (41,2%), et la palpation douloureuse de l’articulation sacroiliaque. La radiographie standard a révélé un aspect flou de l’interligne articulaire, associé à une déminéralisation (68,4%). L’infection a représenté la première cause (82,3%), essentiellement à germes banals (70,58%), ensuite l’arthrose (11,7%), et la spondylarthrite ankylosante (5,9%).
Conclusion. La pathologie sacroiliaque est rare en pratique rhumatologique à Abidjan et concerne les sujets jeunes. Elle est dominée par la sacroiléite infectieuse à germes banals.

Objective and methods. To describe epidemiological and clinical profile of patients with sacroiliac disconfort at The CHU of Cocody during a seven year period, a study of charts of 17 subjects has been conducted. Complaints, clinical examination and imagery findings were screened, and the presumptive aetiology was also a great concern.
Results. The frequency of sacroiliac pathology was 0.55% (17 cases out of 3067 rheumatological consultations), with a female predominance (82.35%), the mean age of the patients was 25.58 years. Gynecoobstetrical events appeared as the major risk factors (47.05%), with inflammatory pain at the buttock or lumbar spine, as the main symptom (64.7%). The pain was described as radiating to the thigh (52.9%) along with a functional disability (82.2%). Fever was reported in 64.7% cases. Positive Tripod maneuver (58.8%), monopodal backing (41.2%), and pain at the palpation of the joint were the main clinical presentations. Standard radiography revealed a blurring of the line spacing associated with demineralization (68.4%). Major etiological factors were infection (82.3%), osteoarthritis (11.7%), and ankylosing spondylitis  (5.9%).
Conclusion. Sacroiliac disease is rare in rheumatology practice in this institution, occurring mainly in young females. The disease is mainly infectious requiring relevant management.
