SummaryRésumé

The kidney of the diabetic is known to be vulnerable to not only diabetic nephropathy but also non diabetic conditions such as interstitial nephropathies, chronic vasculitis, or repeated acute affections (urinary infections and drug intake-related nephrotoxicity and allergies). We report the case of non-diabetic nephropathy in a type 2 diabetic patient known for 3 years with a cardiovascular history, who consults for acute kidney failure in a context of drug toxicity. The etiological exploration by kidney biopsy revealed an aspect of chronic interstitial nephropathy. The present case report is a strong reminder that diabetic patients may develop a non-diabetic nephropathy that requires an etiological treatment beyond the stabilization of blood glucose.

Le rein du diabétique est connu d’être vulnérable sujette non seulement de la néphropathie diabétique qui est délétère par plusieurs mécanismes physiopathologiques (stress oxydatif, rigidité vasculaire, dysfonction endothéliale) mais aussi des affections autre que le diabète telles que les néphropathies interstitielles liées soit aux affections chroniques comme les vascularites, ou affections aigues répétées : infections urinaires et prises médicamenteuses (néphrotoxicité, allergies, produits de contraste). Nous rapportons le cas d’une néphropathie non diabétique chez un patient diabétique type 2 connu depuis 3 ans aux antécédents cardiovasculaires chargés qui a consulté pour une insuffisance rénale d’allure aigue dans un contexte de toxicité médicamenteuse. L’exploration étiologique par biopsie rénale a objectivé un aspect de néphropathie interstitielle chronique. D’où l’intérêt de penser à cette cause ainsi que son traitement étiologique nécessaire à adopter au-delà de l’équilibre glycémique.
