Infection au streptocoque β-hémolytique chez l’enfant : facteur de risque évitable des troubles neurologiques | Β-hemolytic streptococcal infection in children: preventable risk factors for neurological disorders

RésuméSummary

Introduction. Les infections au streptocoque β hémolytique sont fréquentes, elles représentent 20 à 30% des angines chez l’enfant en occident et particulièrement dans nos pays tropicaux où les maladies infectieuses sévissent de manière permanente. Leurs répercussions sur le système nerveux souvent ignorées dans notre milieu sont graves et agissent via plusieurs mécanismes physiopathologiques par le biais soit des cardiopathies, soit des néphropathies, soit par des désordres immunologiques. L’objectif de la présenté étude était d’identifier les complications neurologiques de l’infection au Streptocoque β hémolytique chez l’enfant. Méthodes. Il s’agissait d’une étude des cas de 3 enfants âgés de 11ans, 14 et 6 ayant présenté des complications neurologiques secondaires à une infection à Streptocoque β hémolytique mises en évidence par les bilans neurologique, inflammatoire et la recherche des anticorps antistreptolysines 0. Résultats. Les 3 enfants ont été amenés en consultation pour des mouvements involontaires et avaient eu la fièvre avant l’apparition de ces mouvements. Dans deux cas, il y avait trouble de comportement sous forme d’agitation et d’inhibition psychomotrice. Objectivement il a été observé un syndrome choréo-athétosique, unilatérale gauche dans un cas et, une tachycardie et des vitesses de sédimentation accélérée à 54,56 et 68 mm/1ère heure. Les anticorps antistreptolysines étaient positifs dans 2 cas. La ponction lombaire était normale dans les 3 cas, l’EEG réalisé dans 2 cas indiquait une souffrance cérébrale à prédominance postérieure et le Scanner cérébral obtenu dans 2 cas était normal. L’évolution sous halopéridol, corticoïdes et antibiotiques a été marquée par la rémission des mouvements involontaires dans 3 cas, des séquelles sous forme de cardiopathie mitrale dans un cas et une hémiparésie gauche dans un autre cas. Conclusion. Les infections au Streptocoque β hémolytique, aussi banales qu’elles peuvent être et guérissables, sont susceptibles d’être à la base des complications neurologiques graves via plusieurs mécanismes. Elles nécessitent une prise en charge précoce et adéquate pour la prévention des troubles neurologiques chez l’enfant.

Introduction. b-Hemolytic streptococcal infections are common. They make 20-30% of sore throat in children in Western as well as in Tropical countries where infectious diseases are endemic. The often-neglected repercussions of these infections on the nervous system are severe and involve various pathophysiological mechanisms leading to cardiopathies, nephropathies or immunologic disorders. The objective of the present study was to identify the neurologic complications of β-Hemolytic streptococcal infections in children. Methods. Case study of 3 children aged 11, 14 and 6 years with neurologic complications from a b-Hemolytic streptococcal infection. These complications were assessed via neurologic and inflammatory evaluation as well as the detection of antistreptolysin 0 antibodies. Results. The 3 chidren were taken to a doctor’s consultation for involuntary movements and fever that began before the onset of these movements. In 2 cases, behavioral disorders such as agitation and psychomotor inhibition were observed. In the other case, a unilateral left choreoathetotic syndrome, a tachycardia and an increased erythrocyte sedimentation rates of 54, 56 and 68 mm/1st hour were objectively noticed. Antistreptolysin 0 antibodies were positive in 2 cases. Lumbar puncture sample was normal in 3 cases. The EEG was indicative of a cerebral distress that was predominant on the posterior side; however, the brain CT-scan was normal in 2 cases. Treatment with haloperidol, corticoids and antibiotics led to a remission of involuntary movements in 3 cases, sequelae such as mitral insufficiency in 1 case and a left hemiparesis in the other case. Conclusion. Even apparently mild b-Hemolytic streptococcal infections can lead to severe neurologic complications via various mechanisms. An early and adequate management of these infections is necessary to prevent neurologic disorders in children.
