RésuméSummary

Contexte et objectif.

Le syndrome de lyse tumorale (SLT) est une urgence métabolique, secondaire à la destruction massive, spontanée ou provoquée après chimiothérapie, d’une grande quantité de cellules avec libération de leurs constituants. C‟est une complication caractérisée biologiquement par l‟hyperuricémie, l‟hyperkaliémie, l‟hypocalcémie, et cliniquement par une insuffisance rénale aiguë (IRA). Le traitement standard associe l‟hyperhydratation alcaline et contrôle de l‟hyperuricémie par l‟allopurinol, un uricostatique. Nous évaluons l‟efficacité du rasburicase, un uricolytique, dans le traitement de l‟hyperuricémie du SLT chez 2 patients. Méthodes. Par une approche rétrospective, les dossiers médicaux des deux patients suivis pour lymphome de Burkitt ont été colligés. Résultats: Le premier cas, un garçon de 4 ans, ayant une leucémie aigue lymphoblastique(LAL) de type burkitt. Le SLT s’est développé à l‟admission avant la chimiothérapie. La clinique était marquée par une anémie, et une IR aigue (créatinine à 44mg/l), avec hypertension artérielle. Après une dose unique (0.2 mg/kg) de rasburicase à J5, les niveaux sériques d’urate se sont normalisés dès la 6ème heure du traitement allant de 104mg/l à 0mg/l. la suite thérapeutique a été complétée sans autre incident. Le second cas est un enfant de 15 ans avec un lymphome de burkitt ganglionnaire. Le SLT a été provoqué par une corticothérapie à faible dose compliqué d’IR. La créatininémie à 10.9mg/L et l‟uricémie à 223.9 mg/L. En dépit de la réponse thérapeutique favorable au rasburicase notamment la réduction de moitié d‟uricémie initiale à 1mg/l respectivement à H6 et J2, et un traitement de soutien agressif, ce patient est décédé dans un tableau d‟anasarque.
Discussion: La sensibilité des cellules malignes au traitement dans l‟hémopathie de type Burkitt, pourrait expliquer l’effet cytolytique direct associé au SLT observé chez nos 2 patients ainsi que l‟IR inévitable au début de la chimiothérapie cytoréductrice. Conclusion. Le rasburicase est efficace sur l‟hyperuricémie du SLT malheureusement, sans effet sur une IR préexistante. D‟autres mesures dont la dialyse sont nécessaires pour son mangement. Background and objective.

Tumor lysis syndrome (TLS) is an emergency metabolic disorder secondary to massive cells lysis that occurs spontaneously or in response to chemotherapy, which leads to intracellular compoundsrelease. TLS is characterized by the following laboratory abnormalities: hyperuricemia, hyperkalemia, hypocalcemia, and as clinically damage, acute renal failure (ARF). Standard therapy consists of hydration, and control of hyperuricemia by alkalinization, and use of allopurinol, a xanthine oxidase inhibitor. We evaluated the efficacy of rasburicase, anurate oxidase, as hyperuricemiatreatment, in 2 patients with TLS. Methods. We analyzed retrospectively medical record of two patients having Burkitt lymphoma.
Results: The first patient was 4-year-old with burkitt acute lymphoblastic leukemia, who developed TLS in admission before chemotherapy. Patient presented anemia, ARF (creatinine at 44mg/l), and arterial hypertension. Single 0.2 mg/kg dose of rasburicase, was administered for a serum uric acid level of 104mg/L. Six hours after, the patient’s serum uric acid level decreased to 0 mg/L. After serum urate level normalization, infant completed therapy without incident. The second patient was a 15-day-old man with lymph node burkitt lymphoma who presented TLS in response to corticosteroid few dose complicated by ARF with a creatinine of 10.9mg/L and a uric acid concentration of 223.9 mg/L. Despite favorable therapeutic response to rasburicase, especially decrease of serum uric acid level in half to 1mg/l respectively after 6 hours and 24 hours of administration, as well as aggressive supportive therapy, the infant presented large edema and died. Discussion: The Burkitt malignances cells sensibility in treatment may be at particular risk for TLS in these 2 patients, and inevitable ARF at onset of cytoreductive chemotherapy.
Conclusions. Rasburicase appears to be well effective in lowering serum urate concentrations in the treatment of therapy-related TLS. However, when TLS is complicated by pre-existing ARF, additional interventions as dialysis and alternative anti-tumor strategies may be necessary for a successful outcome.
